Slip: for instructors who
work one position, but split
between mornings and afternoons/evenings.

2012 W-4 Form
Please complete and submit a new form if you would like to change
your federal tax withholding amount or if your name or address has changed. You
may fax it to 920-9518. Please send the entire first page (do not cut).
W-2 Form - Electronic Consent
Please complete and return this form to receive a
password-protected link to annual W-2 Form. Persons who opt for the
electronic version will receive the W-2 Form earlier than those who elect to
receive a paper copy by mail.
Direct Deposit Form
Employee pay will now
be deposited directly into a financial institution account rather than a paper
check. Carefully complete this form and return it to the rec office.
Do not include the check number as part of the account number. If you have
any questions or difficulty, please contact

Certificate of Residency
Each
employee is required to complete and submit this form effective January 1, 2012
in order for West Shore Rec Commission to properly report local Earned Income
Tax (EIT) on behalf of the employee. Please use this
